Maternity Self Referral

Please tell us a bit more about yourself by completing the following form. The form asks questions about yourself and your medical history.  Please answer as fully as possible, as this will ensure that you receive the most appropriate care.

If you are currently taking any regular medications we recommend making an appointment with your GP as soon as possible following a positive pregnancy test to review these.

We also recommend taking a daily dose of Folic Acid 400 micrograms ideally at least 3 months before pregnancy and  a dose of vitamin D 10 micrograms. If you wish to take these as part of a pregnancy multi vitamin tablet make sure they don't contain vitamin A (or retinol). These can be bought at any supermarket or pharmacy. Some women may require a higher dose of folic acid 5mg this can only be prescribed by a GP. For more information regarding this please view both links below

Preconception Advice

Your referral is received immediately via the maternity admin team for processing. A midwife will contact you within 2 weeks to arrange your first booking appointment, which will be between 8-10 weeks of pregnancy.

When you contact us we will ask you for your contact details, which include your name and a method of contact such as your address. This personally identifiable information is stored securely by Taunton and Somerset NHS Foundation Trust and only members of the Maternity Team are able to access this information.

No personally identifiable information provided to TSFT will be passed on or otherwise distributed to any third party (any organisation or person outside TSFT) by TSFT staff except where individual consent has been sought and granted.

Your contact details and information regarding your pregnancy are collected to facilitate our advice and care provision. All information that you provide us with will be treated in the strictest confidence.
