To make a referral to maternity services please complete the following form (see below).

The form asks questions about yourself and your medical history.  Please answer as fully as possible, as this will ensure that you receive the most appropriate care.

If you live within our catchment area for community care but would like to have your baby at another maternity unit (e.g Yeovil, Exeter or Bath) please inform us of this choice when completing the self-referral form as we may need to complete additional paperwork.

Your referral will be processed by the maternity admin team and a midwife will then contact you to arrange an appointment between 8-10 weeks of pregnancy. This appointment is usually held at your local GP surgery or Children’s Centre and your midwife may ask you to make this appointment directly with your registered GP surgery.

The initial booking appointment can last between one to two hours depending on your previous obstetric and personal history, this is to ensure that midwives are able to plan and provide the best care for your pregnancy. Partners and family are welcome to attend the appointment. The midwife will be able to discuss with you any questions or concerns about your pregnancy, book scans, screening tests, and outline when and where you need to attend appointments during your pregnancy.

The Maternity Department complies fully with General Data Protection Regulation (GDPR) and the UK Data Protection Act (2018).

When you contact us we will ask you for your contact details, which include your name and a method of contact such as your address. This personally identifiable information is stored securely by Taunton and Somerset NHS Foundation Trust and only members of the Maternity Team are able to access this information.

No personally identifiable information provided to TSFT will be passed on or otherwise distributed to any third party (any organisation or person outside TSFT) by TSFT staff except where individual consent has been sought and granted.

Your contact details and information regarding your pregnancy are collected to facilitate our advice and care provision. All information that you provide us with will be treated in the strictest confidence.
